Reminders and Escalations

  • Reminder for Level 0/Assessor for upcoming checklist (Similar to Gmeet reminders before 30 min, before 15 min, before 5 min.)
  • Escalation to Reviewer on missed checklist






Deviations: Real-time Intimation and Escalation Matrix in App


Deviations – Real-time intimation of user inputs which are considered deviations.

  1. Real-time deviations:
    • Checkpoints with Feedback – Yes, No, NA.
    • “No” is deviations
  2. Values
    • Out of range in “predefined Value range” is deviations
    • Example: In Refrigerator temperature checklist, 2°C to 8°C is the normal range. Anything above 2°C and 8°C is deviations. Unit of measure is Degree Celsius
    • Example: Water flow meter. It has a normal range (Eg 1-20), and unit of measure is Nm3/hr

Escalations:

  • Deviations are intimated to Level 1 in real time
  • TAT for Level 1 (Reviewer) to manage deviation is 30 min/1 day/2 days (It has to be defined at Form Builder page for the entire Form/Checklist)
  • After TAT, if deviation is not closed in the app, escalation to level 3 (Manager)
  • Escalation to continue with the same logic to Unit Admin



Workflow - Intimation of Deviation to Respective Stakeholders for Recording RCI, CA, PA


  • Checkpoints have 3 options – Yes/No/NA. If an employee clicks “No”, an intimation should go to Level 1/Reviewer as notification in the app. Or if an employee chooses an out-of-normal range then it is a deviation, which is notified to Level 1/Reviewer
    • Time limit of 30 min (time limit will change based on the checklist) for closure (time can be defined during checklist creation)
  • Intimation Card should feature the – Checkpoint statement & Feedback and Remarks box (In case if the employee left any messages in the remarks box).
  • In edit mode, Card should have 3 textboxes
    • Root Cause
    • Corrective Action
    • Preventive Action
  • The responsible employee will complete the 3 text boxes and click completed.
  • The completed card should go to Quality Manager/Unit Admin of the hospital for closure (This step can be built in further iterations)


Workflow: Reminders and Escalations

  • Reminder for Level 0/Assessor for upcoming checklist (Similar to Gmeet reminders before 30 min, before 15 min, before 5 min.)
  • If Level 0/Assessor misses executing the checklist as per schedule, then a real-time escalation message should go to “Level 1/Reviewer”.
  • If the checklist is not executed even after 30 min, the next level escalation to “Level2/Manager”
